Eligible seniors should download and complete this application to be considered for one of several scholarships awarded by the Rice Lake Sports Hall of Fame each year.
Use this form to apply for or renew membership in the Rice Lake Sports Hall of Fame. Must be renewed annually.
Members use this form to submit nominations to be considered for induction into the hall.
